If your insurance determines that services rendered are “medically unnecessary” we will bill you for any outstanding charges.

We may recommend an elective procedure which is not covered by your health care service plan. You are responsible for paying for it. We will provide you with a notice to help you make an informed choice about whether or not you want to receive these service, knowing that you may have to pay for it yourself. Before you make a decision about your options, you should read this entire notice carefully. Ask us to explain if you don’t understand why your health care service plan may not pay.
